This form is for PARENTS requesting an early dismissal because:

The student is a senior with a Woodside paid parking decal who needs to drive off campus. ONLY SENIORS may drive off campus during school hours.
The person picking up the student is not on the student’s emergency contact list. (If you wish to change your student’s emergency contact list, please contact the school counseling office.)
